The Traditional Path vs the Telehealth Path
Traditional: schedule with your primary care doctor, ask for a referral to an obesity medicine specialist or endocrinologist, wait weeks to months for an appointment, get evaluated, get a prescription, navigate the pharmacy and insurance, potentially wait for stock.
Telehealth: complete an online intake form covering your health history and weight loss goals, have a consultation with a licensed provider (often async, sometimes video), receive a prescription if you qualify, get medication shipped to your door from a licensed pharmacy.
Both paths can work. The traditional path is appropriate if you have a complex health history that needs in-person evaluation. The telehealth path is faster and cheaper for most otherwise-healthy adults who can’t afford to wait months for an appointment.
Do You Qualify?
Most GLP-1 programs look for one of two things:
- BMI of 30 or above (classified as obesity)
- BMI of 27 or above with at least one weight-related condition: high blood pressure, type 2 diabetes, high cholesterol, or sleep apnea
These are the same criteria used in the FDA approvals. Individual telehealth programs may vary slightly. Most offer a free eligibility check before you commit to anything.
What the Online Consultation Covers
Expect to answer questions about your medical history, current medications, any allergies, previous weight loss attempts, and your goals. Programs also ask about contraindications: personal or family history of thyroid cancer or MEN2 syndrome, which are reasons GLP-1 medications are not prescribed.
The provider reviews your intake, may ask follow-up questions, and either approves you for treatment or explains why you’re not a candidate.
Compounded vs Brand-Name
Most telehealth programs offering affordable GLP-1 medications use compounded semaglutide or tirzepatide, not brand-name Ozempic, Wegovy, Mounjaro, or Zepbound. Compounded medications are made by licensed pharmacies and are not FDA-approved, though the active ingredient is the same.
This is why telehealth programs can price significantly below brand-name retail. Compounded versions are not identical to brand-name products, but the clinical difference for most patients is minimal and the cost difference is substantial.
What to Look for in a Program
- Licensed providers: The prescribing doctor or NP must be licensed in your state.
- Licensed pharmacy: Medication should come from an accredited compounding pharmacy.
- Transparent pricing: You should know what you’re paying before you commit. Watch for low intro prices that jump significantly in month two.
- Medical support: Look for programs that include check-ins or have a way to contact a provider if you have questions or side effect concerns.
- Clear cancellation terms: Some programs auto-renew without clear cancellation options. Read the terms before entering payment information.
