Wegovy Weight Loss: What the FDA Label and Trials Show

Wegovy weight loss in the main clinical trial, STEP 1, averaged 14.9 percent of body weight over 68 weeks, compared with 2.4 percent on placebo, in 1,961 adults without diabetes who also had lifestyle support (Wilding et al., New England Journal of Medicine, 2021, funded by Novo Nordisk). That is a trial average, not a promise, and Wegovy is a prescription medication with a boxed warning, so your health history matters. Below is what the FDA label says, what each trial measured, what happened when people stopped, and what the label says about pregnancy.

Is Wegovy a weight loss drug?

Yes. The current FDA label (revised February 2026) says Wegovy (semaglutide) is indicated, with a reduced-calorie diet and increased physical activity, to reduce excess body weight and maintain weight reduction long term in adults with obesity, or in adults with overweight who have at least one weight-related condition. It also lists lowering the risk of major cardiovascular events in adults with established cardiovascular disease and either obesity or overweight, and treating noncirrhotic MASH, a liver disease, with moderate to advanced fibrosis.

Wegovy comes as a once-weekly injection in a prefilled single-dose pen (the label’s dosage forms section) and as once-daily tablets. Novo Nordisk’s Wegovy site (read October 22, 2026) describes the tablet as a once-daily pill, and the label says using Wegovy with other semaglutide products or any other GLP-1 receptor agonist is not recommended.

Who is Wegovy for under the FDA label?

The indication says obesity, or overweight with a weight-related condition. The trials defined those as a BMI of 30 or higher, or a BMI of 27 to 29.9 plus a condition such as treated or untreated high cholesterol or high blood pressure, per the label’s clinical studies section. Most weight trials excluded type 2 diabetes, and one, STEP 2, enrolled only people who had it.

The label says Wegovy is contraindicated with a personal or family history of medullary thyroid carcinoma, with Multiple Endocrine Neoplasia syndrome type 2, or with a known allergy to semaglutide or its ingredients.

How does Wegovy work for weight loss?

This medication acts on appetite signaling. The label’s mechanism section says semaglutide is a GLP-1 analogue, that GLP-1 regulates appetite and calorie intake, and that animal studies showed it activating brain regions involved in food intake. The pharmacodynamics section adds that semaglutide lowers body weight with greater fat mass loss than lean mass loss and decreases calorie intake, likely by affecting appetite.

How successful is Wegovy for weight loss?

Here are the main trials, showing average change in body weight from the start versus placebo. Each paired the medicine or placebo with lifestyle support.

In STEP 1, 86.4 percent of the semaglutide group lost at least 5 percent of body weight versus 31.5 percent on placebo, and 50.5 percent lost at least 15 percent versus 4.9 percent. The label’s table for the same trial lists somewhat lower shares, 83.5 and 47.9 percent. STEP 1 and STEP 2 list Novo Nordisk as funder, and company employees are among the authors of STEP 1 and STEP 5, so these are industry-sponsored results.

How much weight do women lose on Wegovy?

The trials enrolled mostly women: the label says 74 percent of STEP 1 participants were female, with a mean age of 46. But the label reports weight results for each trial as a whole, not by sex or menopause status, so I can’t give you a women-only or over-50 average. Groups close to this site’s readers in age and sex lost meaningful weight on average, and individual results vary.

What dose of Wegovy is used for weight loss?

Your prescriber chooses your dose, not a website. The label describes the injection as starting at 0.25 mg once a week for 4 weeks, then stepping up every 4 weeks to 0.5 mg, 1 mg and 1.7 mg. From week 17 the maintenance dose for weight reduction is 2.4 mg, which the label calls recommended, or 1.7 mg. It says the slow steps are meant to reduce stomach side effects, and that delaying an increase by 4 weeks can be considered if a dose isn’t tolerated.

The tablet starts at 1.5 mg once daily for 30 days and steps up every 30 days to a 25 mg maintenance dose. The label says to take it on an empty stomach in the morning with up to 4 ounces of water and wait at least 30 minutes before eating, drinking or taking other oral medicines.

Does the Wegovy pill work for weight loss?

In the label’s Study 7, 307 adults without diabetes took the 25 mg tablet or placebo for 64 weeks, and 79 percent were women. Average body weight fell 13.6 percent on the tablet versus 2.4 percent on placebo, and 76.3 percent of the tablet group lost at least 5 percent versus 31.3 percent. The pill and injection were tested in separate trials of different lengths and sizes, so please don’t read those percentages as a head-to-head comparison.

What happens when you stop Wegovy?

In the trials that tested it, much of the lost weight came back. In STEP 4, 803 adults who had reached the 2.4 mg dose over 20 weeks were randomized to continue semaglutide or switch to placebo for 48 more weeks, and 79 percent were women. From week 20 to week 68, body weight changed by minus 7.9 percent when they continued and plus 6.9 percent when they switched.

The STEP 1 extension followed 327 participants for a year after both semaglutide and the lifestyle program stopped. They regained 11.6 percentage points of the 17.3 percent they had lost, about two-thirds, and cardiometabolic improvements moved back toward baseline for most measures. The authors called these analyses exploratory.

What are the side effects of Wegovy?

Digestive effects were the most common. This table is from the label, pooling three placebo-controlled trials with 2,116 people on Wegovy 2.4 mg and 1,261 on placebo. Of the people on Wegovy, 71 percent were women.

What serious warnings does the Wegovy label include?

Wegovy’s label puts a boxed warning about thyroid C-cell tumors at the top. Semaglutide caused them in rodents at clinically relevant exposures, and it is unknown whether Wegovy causes them in humans. The warnings section also lists acute pancreatitis, gallbladder disease, low blood sugar when combined with insulin or an insulin secretagogue, acute kidney injury from volume depletion, severe gastrointestinal reactions, allergic reactions, diabetic retinopathy complications, increased heart rate, and pulmonary aspiration during anesthesia, so tell your providers about planned procedures.

Is Wegovy safe during pregnancy?

The label says weight loss offers no benefit to a pregnant patient and may cause fetal harm, and that Wegovy should be discontinued when pregnancy is recognized. Because semaglutide has a long half-life, it says to discontinue at least 2 months before a planned pregnancy. It also notes that available data are insufficient to establish a risk of birth defects or miscarriage. If you could become pregnant, raise this with your prescriber first.

What did the heart trial report about fractures in women?

In the cardiovascular outcomes trial, the label reports more hip and pelvis fractures on Wegovy than placebo in female patients, 1 percent (24 of 2,448) versus 0.2 percent (5 of 2,424). It does not say why. Bone health is a fair question for your clinician.

Does Wegovy lower the risk of heart attack and stroke?

For adults with existing heart disease, it did in one large trial. SELECT enrolled 17,604 adults aged 45 or older with established cardiovascular disease and a BMI of 27 or higher, none with diabetes. Over a mean 39.8 months, a major cardiovascular event occurred in 6.5 percent of the semaglutide group and 8.0 percent of the placebo group (hazard ratio 0.80). Novo Nordisk funded SELECT, and the label says 72 percent of participants were men, so it tells us less about women without heart disease.

Is compounded semaglutide the same as Wegovy?

No. Wegovy is the FDA-approved product. The FDA’s page on unapproved GLP-1 drugs used for weight loss (current as of October 1, 2026) says the agency does not review compounded drugs for safety, effectiveness or quality before they are marketed, and it had received 990 adverse event reports tied to compounded semaglutide as of May 31, 2026, likely an undercount.
