Wegovy before and after results in a main clinical trial averaged a 14.9% loss of body weight over 68 weeks, compared with 2.4% on placebo, in a group that was 74% women (STEP 1 trial, New England Journal of Medicine; Wegovy label). That is an average across women and men, and the spread around it is wide. Below I show the trial numbers in real pounds, who was actually studied, how my own result compares, and what the side effects and the regain data look like.

I am Judy, and I am not a doctor or a registered dietitian. I put my own story next to the typical trial result on purpose, so you can see how far one woman’s experience sits from the average.

What Do Wegovy Before and After Results Look Like for Women in the Trials?

The largest of the trials behind the weight loss approval was called STEP 1. It randomized 1,961 adults who had a body mass index of 30 or higher, or 27 or higher with at least one weight-related condition such as high blood pressure, and who did not have diabetes. Participants averaged 46 years old and about 105 kg (232 lb), 74% were female, and everyone also got lifestyle counseling (Wegovy label, clinical studies section).

On Wegovy 2.4 mg once a week, the group lost an average of 14.9% of body weight by week 68, versus 2.4% on placebo. In the journal paper that works out to an estimated 15.3 kg (about 34 lb) on Wegovy versus 2.6 kg (about 6 lb) on placebo (PubMed abstract).

How Many Women Reached 5%, 10%, 15% or 20% Weight Loss?

The label reports the share of all randomized participants who reached each level. These figures include people who stopped treatment early, and they cover women and men together:

Weight reduction at 68 weeks Wegovy 2.4 mg Placebo
5% or more 83.5% 31.1%
10% or more 66.1% 12.0%
15% or more 47.9% 4.8%
20% or more 30.2% 1.7%

Put plainly, about one in six treated participants did not reach a 5% loss, and about three in ten lost 20% or more. The journal paper reports somewhat different shares for the treated group (86.4%, 69.1% and 50.5%), so check which analysis any page is quoting before you compare numbers.

What Does a 14.9% Average Loss Mean in Pounds?

A 14.9% loss works out to roughly 27 lb for a woman starting at 180 lb, 33 lb at 220 lb and 39 lb at 260 lb. That is my own arithmetic applied to the trial average, not a figure the trial reported for each weight. A conference analysis of the same trial found that a lower starting weight was linked to a greater percentage response, so treat these as a rough picture of the middle of the range (STEP 1 subgroup abstract, Journal of the Endocrine Society).

Do Women Lose More Weight Than Men on Wegovy?

On average, a little more, according to exploratory analyses. A post hoc analysis of STEP 1, published as a conference abstract, reported that average weight loss at week 68 was greater in women than in men, and the abstract gave no separate figures for each sex (Journal of the Endocrine Society). The STEP 1 extension study also saw smaller average losses in men, and its authors called all of its analyses exploratory (Diabetes, Obesity and Metabolism). An average cannot tell you where you will land, and both sexes lost a meaningful amount.

How Do Wegovy Results Differ in Other Trial Groups?

The result depends on who was studied, so here are the other main trials, each with its own population:

Women and Men With Type 2 Diabetes (STEP 2)

STEP 2 enrolled 1,210 adults with type 2 diabetes and a body mass index of 27 or higher, The label reports 51% women in its 807-patient comparison of 2.4 mg and placebo. The 2.4 mg group lost an average of 9.6% versus 3.4% on placebo at 68 weeks, a smaller loss than in people without diabetes (STEP 2, The Lancet).

Adults in Intensive Lifestyle Therapy (STEP 3)

STEP 3 enrolled 611 adults without diabetes, 81% of them women, and added an 8 week low-calorie diet and 30 counseling visits. The Wegovy group lost an average of 16.0% versus 5.7% on placebo at 68 weeks (STEP 3, JAMA). That was a heavily supported program, which is worth remembering when you read the number.

What About Menopause?

The participants in STEP 1 averaged 46 years old, and the sources linked on this page do not break results out by menopause status. If weight gain around menopause is your situation, a prescriber who knows your hormone and health history is the right person to weigh what the trials do and do not tell you.

How Does My Own Result Compare With the Trial Average?

My experience, not medical advice. I was 47 and about 68 pounds overweight when I started GLP-1 injections in May 2024, and in 14 months I lost 31 lbs (about Judy). That is one woman’s result on a GLP-1 injection, not a Wegovy result. I tell the longer story in Ozempic for weight loss.

The typical trial result, side by side: the STEP 1 group averaged roughly 34 lb (14.9%) over 68 weeks, which is about 16 months. So 31 lb in 14 months is a little under the 34 lb trial average, but pounds are a poor comparison across different starting weights. The table above shows plenty of participants well above and well below the average, and I am simply one data point.

What Is a Wegovy Timeline by Dose?

Wegovy injections start at 0.25 mg once a week and step up every four weeks: 0.5 mg in weeks 5 to 8, 1 mg in weeks 9 to 12 and 1.7 mg in weeks 13 to 16. From week 17 the maintenance dose is either 1.7 mg or 2.4 mg, with 2.4 mg recommended (Wegovy label, dosage section). The 14.9% average was measured at week 68, after 52 weeks at the maintenance dose, so the early months look smaller on the scale. I map the first year in how long Ozempic takes to work, which covers the same drug class.

Do You Gain the Weight Back After Stopping Wegovy?

On average, a large share comes back. In the STEP 1 extension, 327 participants (67% female; 228 who had taken Wegovy and 99 who had taken placebo) stopped treatment and lifestyle support after 68 weeks. The Wegovy group had lost 17.3% on average, regained 11.6 percentage points of it by week 120 and kept a net 5.6% loss, which the authors summarized as about two-thirds of the loss regained. This was a subset of sites, and all analyses were exploratory (STEP 1 extension).

STEP 4 points the same way. After a 20 week run-in, 803 participants (79% women) were randomized to continue Wegovy or switch to placebo. Over the next 48 weeks the continuing group lost a further 7.9% while the placebo group regained 6.9% (STEP 4, JAMA). This is the part before and after photos never show, and it is why I treat the medication as a long-term plan to make with a prescriber.

What Are the Side Effects of Wegovy for Women?

The adverse reactions on the label are mostly digestive. In the weight-loss trials, nausea affected 44% of Wegovy patients versus 16% on placebo, diarrhea 30% versus 16%, vomiting 24% versus 6%, constipation 24% versus 11% and abdominal pain 20% versus 10%. Those trials were 71% female, and 6.8% of Wegovy patients stopped because of side effects versus 3.2% on placebo (Wegovy label, adverse reactions).

Does Wegovy Cause Hair Loss in Women?

It can. In the pooled trials, hair loss was reported by 3.3% of patients on Wegovy 2.4 mg, which was 4% of women and 0.9% of men, against 1% on placebo (2% of women). The label ties it to weight reduction (Wegovy label, hair loss). I cover what to expect and when to call your prescriber in Ozempic side effects.

Who Should Not Take Wegovy?

Wegovy is contraindicated with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2, and after a serious allergic reaction to semaglutide. The label carries a boxed warning about thyroid C-cell tumors seen in rodents, and it warns about pancreatitis and gallbladder disease. For women of childbearing age, the label says to stop Wegovy when pregnancy is recognized and to discontinue it at least 2 months before a planned pregnancy (Wegovy label).

Who Is Wegovy Approved For?

For weight reduction in adults, the label lists Wegovy, with a reduced-calorie diet and more physical activity, for adults with obesity and for adults with overweight who have at least one weight-related condition (Wegovy label, indications). Whether it fits you is a prescriber’s call, based on your health history and your other medicines.

How Much Does Wegovy Cost Without Insurance?

When I read the NovoCare Pharmacy page on October 14, 2026, it listed new self-pay patients at $199 a month for the first two fills of the 0.25 mg and 0.5 mg doses through December 31, 2026, then $349 a month for doses up to 2.4 mg, and it says the program can change (NovoCare Pharmacy). I break the options down in how much Ozempic and Wegovy cost.

Where Can You Get Wegovy Through Telehealth?

If you decide to try telehealth, a licensed clinician decides whether treatment is right for you. On October 14, 2026, the GobyMeds home page listed Wegovy and Zepbound next to compounded semaglutide and compounded tirzepatide under weight loss, and said treatment is only provided if a licensed provider determines it may be appropriate after review. ShedRX listed a weekly Wegovy pen alongside compounded semaglutide injections. GobyMeds uses one general registration page for all of its products, so this link opens an account sign-up, not a page about one treatment. After you create an account, you can start a consultation for the treatment you want.

The FDA says compounded drugs are not FDA approved and are not reviewed for safety, effectiveness or quality before they are marketed, so ask which product you are being prescribed (FDA). My comparison is in how to get GLP-1 online, and my notes on one option are in my GobyMeds review.

Medical disclaimer: I am a woman who writes about her own experience and the published research, not a doctor, and nothing here is a prescription or a diagnosis. Talk to your prescriber about your own health history before you start, change or stop any weight loss medication.

Frequently Asked Questions

How long does it take to see Wegovy results?

The dose steps up for 16 weeks before the maintenance dose, and the trial average of 14.9% was measured at week 68. Your own timeline depends on your dose steps, how you tolerate each one and how you respond.

Are Wegovy before and after photos reliable?

A photo shows one person at one moment, with no dose, timeline, diet or setbacks attached. Trial averages and the full spread of results are a better guide to what to expect, and a single dramatic photo is the best case rather than the typical one.

Do you still have to change your diet and exercise on Wegovy?

Yes. The label describes Wegovy as a treatment used together with a reduced-calorie diet and increased physical activity, and STEP 1 participants received lifestyle counseling with an advised deficit of about 500 calories a day and at least 150 minutes of activity a week.

Is the Wegovy pill as effective as the injection?

The two were tested in different trials, so the numbers cannot be compared directly. The 25 mg tablet group lost an average of 13.6% versus 2.4% on placebo at 64 weeks, in a trial that was 79% women, while the injection group in STEP 1 lost 14.9% at 68 weeks. The tablet is taken daily on an empty stomach with up to 4 ounces of water, followed by a 30 minute wait before eating or drinking.

What is the maximum dose of Wegovy?

The label allows a higher 7.2 mg weekly dose for patients who tolerate 2.4 mg for at least four weeks and need more weight reduction. In a 72 week trial of adults with obesity and no diabetes, 7.2 mg produced an average loss of 18.8% versus 3.9% on placebo.

Does Wegovy work for women with menopause weight gain?

The trials did not report menopause as its own group in the sources linked here. Women were the majority of participants and lost weight on average, and your prescriber can weigh your hormone and health history.
